Help offered in Poole for families facing care transition

Families of disabled children in Dorset can get expert guidance on the move to adult care services at a support session in Poole this month.

Diverse Abilities, the county’s disability charity, is hosting a transition advice morning on Wednesday 25 June at Bourne View in Langside Avenue.

The event is aimed at parents and carers of young people approaching adulthood who are likely to face significant changes to their health and social care support.

The drop-in will include professionals from BCP Council, the NHS and other local charities, as well as staff from Diverse Abilities. It is designed to provide practical help and reassurance for those navigating this often stressful and complex period.

Families attending will also be able to connect with others going through similar experiences.

Lynda Larke from Diverse Abilities said: “It’s great to get families together and help them to get the support they need. When children reach 18, their families are at a cliff edge, everything they have known in terms of funding and healthcare provisions ends, and the process begins all over again.”

The event starts at 10.30am and families can submit questions or register their interest at diverseabilities.org.uk/transitionsdropin.
